We provide our services to diverse communities/boroughs Barnet, Brent, Hammersmith & Fulham, Kensington and Chelsea, Westminster, Merton and Harrow, and from 1st October we are now delivering a range of adult services in West Hertfordshire.

Locality Manager - Dacorum

This is an exciting time to join the community services in West Herts. We are developing new community pathways which will support the delivery of more care in people’s homes, and are working closely with colleagues in primary care and partner organisations to deliver the aspirations of the Herts and West Essex STP and the Local Delivery Plans. We are looking for an experienced operational manager with a clinical qualification to lead the Hertsmere Locality team.

The Dacorum Locality is an integrated team of community nurses, physiotherapists, occupational therapists, paramedics, and support staff, delivering care to patients in their own homes with the aim of maximising independence and self care.

Louise is the Clinical Business Unit Manager for Planned Care. She has worked in community services in Hertfordshire for 8 years and for the last two years has led the team of four locality managers in West Herts. She says:

“It has been an eventful couple of years in community services, with the development of Primary Care Networks, a collaborative Locality Delivery Programme, CQC Inspection (we are Good!) and the transfer of services from HCT to CLCH. I am so incredibly proud of everything that the Locality Leadership Team has achieved during this period. They have been consistently positive and supportive and with their passion and tenacity it has been possible to maintain the service, support our staff, and continue to deliver great care to our patients. I am delighted that members of the team have had new career development opportunities, but this means that we need to recruit new members to the team. That is where you come in! The challenge isn’t over, we have frailty services to launch, relationships with Primary Care Networks to build, and new pathways to embed. Our Dacorum team is based at Hemel One, in Hemel Hempstead, just off the M1 at Junction 7.
