The Northern Care Alliance NHS Group (NCA) unites five local hospitals to deliver high quality care across the North East of Greater Manchester.

Heartly Green is in Irlam and is a 29 bedded unit.

We are looking for registered nurses to oversee the patient journey for patients within the unit.

This is an exciting opportunity to be part of the integration between Social and Healthcare, with the aim of delivering high standards of quality care and therapy to enable our patients to return home or to their preferred place of residence as soon as possible.

You will be supported by GP’s, Consultants, Nurses, Allied Health Professionals and carers. The unit provides high quality comprehensive care to patients for rehabilitation and also assessment for 24 hour care.

The majority of patients within the bedded units are frail older people so experience of and a commitment to working with this patient group is essential. To help you achieve this you will be fully supported by our GP’s from Salford Care Homes Medical practice and our Geriatricians.

You will also have access comprehensive development opportunities aided by our Quality Improvement team and also the opportunity to influence care within in the Integrated Care Organization of which the Trust has received Vanguard status.
