An exciting opportunity has arisen for the position of a MDT Co-ordinator/ Administrator within our Discharge to Assess team Livewell South West.

The Discharge to Assess team are looking for a highly motivated and enthusiastic individual to join them in providing 'behind the scenes' support to patients who have been referred to a multi-disciplinary team follow a period in hospital.

The role involves a variety of responsibilities to include providing support and facilitate MDT meetings, related administration working to guide the patient through the DTA pathway in line with local targets.

Excellent communication and interpersonal skills are essential as you will be required to liaise with staff at all levels associated with multi-disciplinary team working. You will need to be able to work well as part of a larger team as well as have the ability to work autonomously and be able to multi-task.

You should be able to demonstrate attention to detail; exceptional organisational skills and be able to manage changing priorities as they arise, in a calm and professional manner to ensure the smooth running and efficient service.

Tier 2 Certificate of Sponsorship
