We are looking to recruit an Advanced Nurse Practitioner to cover maternity leave for a period of 6 months to work in the Intermediate Care Unit in Salford

Main purpose of the job:

To work with an extended team of clinicians including GP’s, Consultants, Nurses, and Allied Health Professionals in this innovative service caring for patients in three Intermediate Care Bedded units across Salford.

The post holder will:

  • Demonstrate an ability to work across the interface between primary and secondary care in a service whose core users are frail older people, many of whom have cognitive impairment.

 

  • Be the first point of contact for patients, including on admission to the unit and if they become unwell.

 

  • Work with the multidisciplinary team to ensure that patients receive a high quality comprehensive geriatric assessment

 

  • Demonstrate advanced skills and competencies, including interpretation of presenting symptoms, physical findings and investigations to enable diagnosis and management of patients in the context of a mainly frail older patient population.

 

  • Ensure that evidence based practice is inherent in all aspects of care and treatment and ensure this knowledge is disseminated throughout the clinical area, Trust, and externally as appropriate.

 

  • Provide specialist input as an independent and interdependent practitioner developing and leading relationships with members of the multi-professional team.

 

  • Communicate with specialist services to facilitate appropriate management of the patient

 

  • Play a leading role in service development in line with the local and national agendas.

 

• Undertake comprehensive medication reviews for patients with multiple complex co-morbidities with the support of other healthcare professionals
