Social Prescribing Link Worker for York Road Primary Care Network

We are looking for an enthusiastic Social Prescribing Link Worker to join our York Road Primary Care Network of 3 GP Practices

We are looking for a patient facing team of Social Prescribers with the experience, communication and knowledge of our local area to join our multi-disciplinary General Practice teams

This is an excellent opportunity to work directly with people with more complex needs and help them to access the support they need to live independent and healthier lives


About the role

Social Prescribing empowers people to take control of their health and wellbeing through referral or direct access to non-medical ‘link workers’ who give time and focus on ‘What matters to me’ and take a holistic approach, connecting people to the services and community groups for practical and emotional support

As a Social Prescriber link worker you will provide 1:1 support to people who are referred to you by team members and local agencies, understanding their needs and helping them access the support they need to achieve their goals.

This will involve building links and liaising with various services/staff in the local area, working closely with the GP Practices, social care, voluntary and community organisations

The Social Prescribing link worker will build trusting relationships with people, understanding their real needs, creating and tailoring self-care plans and support plans, and connecting them to the appropriate social care, voluntary and community organisations which will empower the patient to make the necessary changes to enable them to live the life the choose

This is a fantastic opportunity to become part of a team within our Primary Care Network that will develop and improve patient care and exercise a flexible and proactive approach, working as part of a multi-disciplinary team

  • You must be a good listener, have time for people and be committed to supporting local communities to care for each other
  • You should have experience of working positively with people facing complex social and emotional challenges
  • You will have great interpersonal skills in supporting people, community groups and local organisations
  • You will ned to be able to put people from all walks of life at their ease and inspire confidence
  • You will have experience of building an individual’s motivation, confidence and be able to connect them with appropriate community services through supporting them to achieve their goals
  • Motivational interviewing or health coaching type skills would be useful, as would a sound knowledge of the support resources available to the people in the area along York Road
  • You will also need good organisational skills and an understanding of priorities along with being able to manage your own caseload effectively
