The Health Case Management Service provides a case management function for the population of Leeds (18 years and above) who are eligible for NHS Fast Track and Continuing Healthcare funding; this includes hospital, hospice, care homes and within their own home, over 7 days, Monday to Friday 08:00 to 20:00 and Saturday and Sunday 09:00 to 17:00.

The Health Case Management Service is a citywide service, divided into two areas (North and South). The teams consists of Health Case Managers and Health Case Management Assistants who are professionals with health and social care backgrounds and have a wealth of experience in all aspects of health case management.

The band 5 opportunity is welcoming candidates who exhibit the values and behaviours of Leeds Community Healthcare and can demonstrate attributes required for the role; day to day coordination responsibilities which include assessment, delivery and evalua tion of person centred care within your domain of competence and knowledge. We encourage applicants who hold an NVQ Level 4 professional degree or equivalent, have a knowledge of health and social care in particular around the service area and profession, along with being able to demonstrate a wide variety of communication skills and who has experience of working as part of a wide multi-disciplinary team.

You will be supported by line management of Clinical Quality Leads; this will be through regular supervision and the appraisal process which includes continuous professional development.

Due to the nature of the role it is essential that applicants are able to travel independently across the city. If necessary adjustments can be considered in accordance with the Equality Act 2010.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.43 or Standard £30.43) from your salary. There will be no charge if you are enrolled in the DBS Update Service or a current employee of the Trust.
