Cardiac Rehabilitation Physiotherapist  

Heywood, Middleton & Rochdale

  •       Planning, co-ordinating and delivering a patient-centred cardiac rehabilitation programmes across designated sites within the CCG using current evidence-based practice.
  •       Undertaking comprehensive subjective and objective assessments of cardiac patients and identifying when further clinical support is required.
  •       Communicating effectively and working collaboratively with colleagues to ensure delivery of a co-ordinated service.
