We are looking for experienced Community Nurses who want a new challenge to join our hardworking and dynamic team in Wandsworth. We aim to ensure that quality is at the heart of everything we do ensuring that we have well resourced, well-structured, thoughtful and efficient services run by staff that care passionately about the patients we serve. The CQC rated us as good for the care that we provide and outstanding in adult community services in the well led domain. We were also awarded ‘organisation of the year’ by the Health Service journal for our Quality Strategy and specifically, for our model of shared governance where staff identify and collaboratively make changes to the way in which we deliver care.

As the Band 7 District Nursing Team Leader, you will be leading and managing a community nursing team, responsible for the provision of excellent community care to a diverse population of patients. We aim to facilitate self-care for our patients working in partnership with them and a number of partners to manage their long term conditions and to support their health promotion using innovative activities and projects to enhance patient experience. We are looking for an individual with experience in community nursing and a post–registration qualification in district nursing or equivalent. You should also have experience of negotiating and liaising within a multi-disciplinary team and experience of working with people with complex needs.
