A rare and exciting opportunity has arisen within the Twilight team for Central Overnight Responsive Services.

We are looking for a Band 3 Healthcare Assistant to help support patients in their own homes between the hours of 18:00 and 23:00, based within our Haywards Heath team. The main focus of the job is to help prevent avoidable hospital admissions.

We are able to provide a supportive environment, flexible working hours and all the training required for your role.

The Twilight team are a responsive service that predominantly responds to urgent nursing needs and admission avoidance referrals. We also provide short term rehabilitation to patients following a stay in hospital where required in the evenings.

The successful applicant must have their own car and have a full UK driving licence.
