An opportunity has arisen within the Responsive Services team in Brighton and Hove for a highly skilled, motivated and enthusiastic individual to join our team. Based at Southfield House in Worthing's town centre, we deliver services across the Worthing area and within patients' normal places of residence.

As a Senior Nurse you will join our multidisciplinary team which consists of an Advanced Nurse Practitioner, Nurses, Occupational Therapists, Physiotherapists, Registered Mental Health Nurses, Social Workers, Pharmacists, Community Reablement Assistants, and Community Care Assistants. Working collaboratively with our acute Trust and other community services including third sector organisations, we aim to improve the patient journey and pathways of care and deliver short term intervention, care and support in the community.

Responsive Services delivers the ‘right care at the right time in the right place’, playing a key role in supporting patients' discharge from hospital and community beds and supporting patients to stay at home and prevent avoidable admission to hospital.

The successful candidate must hold a full UK driving licence, with access to a car for work purposes.
