An exciting opportunity to work as part of the multidisciplinary community team in Worthing, coordinating and delegating individual patient care by working across boundaries within secondary and social care settings. The Clinical Nurse Lead role uses advanced clinical skills and expert knowledge to assess, diagnose and treat the physical and psycho-social needs of a patient group, instigating therapeutic treatments based on best available evidence in order to improve health outcomes.

You will also provide clinical leadership to the community nursing teams through clinical advice and support in caseload management. You will be able to challenge professional and organisational boundaries, identifying areas for skill/knowledge development and apply these to practice to provide continuity and high quality patient-centred healthcare.

Responsive Services is a multidisciplinary team providing a seven day service between the hours of 8:00am and 8:00pm. It provides:

  • A rapid response to people in the community who are experiencing acute episodes of ill-health
  • A supported discharge service to facilitate timely hospital discharges
  • Short term support for people who require assistance with activities of daily living
  • A rehabilitation service including a range of therapies
  • The Occupational Therapy service to the local A&E department

The service is looking for a Band 7 Clinical Nurse Lead to provide clinical leadership to the nurses within the team. The emphasis will be on providing clinical expertise and advice for patients under the care of the team. You will be expected to support skill development of both registered and unregistered team members, keeping clinical practice up to date and to a high standard. The post holder will need to have experience of working within a variety of clinical settings, particularly the community and acute settings. The post holder will need to demonstrate advanced clinical assessment skills, and a nurse prescribing qualification would be an advantage.

  • Participate and assist in the implementation of the Calderdale Framework
  • Supervise and support development of other staff outside of your professional group
  • Develop skills beyond the usual scope of practice of your profession
  • Assist in shaping future clinical practice of the nursing team
