The Brighton and Hove Community Nursing team are looking to recruit an Operational Team Lead. You will need to be a highly skilled, motivated and enthusiastic individual with a passion for leadership who can demonstrate a wide range of skills and experience.

You will be responsible for ensuring the Community Nursing team deliver the "right care at the right time in the right place" and join a well-established team made up of Clinical Nurse Leads, Care Coordinators, Nurses, Assistant Practitioners, Healthcare Assistants and Allied Health Professionals, all of whom work in partnership with secondary care, primary care and third sector organisations to deliver services that provide holistic care to the frail, elderly, and people unable to leave their homes.

The team works closely with all specialist teams to help patients manage long term health conditions that cannot be cured with therapy and/or medication.
