About Barnet Federated GPs

Barnet Federated GPs CIC is a not for profit organisation, putting GPs and healthcare professionals at the forefront of Primary Healthcare provision in Barnet. The Federation is building a large team who will work as one in developing Primary Care Services in Barnet and will support Primary Care Networks.


The Federation was formed in 2015 and aims to improve the quality of health in Barnet through sustainable primary care. Its current membership consists of all 52 primary care practices within the borough. Although we are a private company, we hold a strong affiliation to the NHS, its constitution and guiding principles.


The Role

We are excited to create this new role of Social Prescribing Service Manager to support the Primary Care Networks in Barnet to set up and manage the pan Barnet Social Prescribing Service. Leading on this work and working in partnership to support the delivery of the integrated Social Prescribing Service model, the post holder will build upon our current offer and develop innovative proposals to deliver social prescribing and wellbeing services across Barnet. The post holder will also build relationships with local health, care and voluntary sector partners to maximise the benefit of shared knowledge and interventions.

  • You will be working in partnership with local voluntary and community services and agencies, ensuring the best outcome for patients.

The Candidate

The successful candidate will lead the Barnet social prescribing team that will be working with the 7 Primary Care Networks in Barnet, supporting the General Practices and working with people who have long-term conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing.

The successful candidate will be able to demonstrate the ability to be a good listener, have time for people and be committed to supporting local communities to care for each other. You will have team management and supervision experience.

You will have a critical role in developing and supporting a team of eight new Link Workers, quality assuring and monitoring the delivery of social prescribing service, liaising with the new Primary Care Networks and building positive relations with a wide network of voluntary, community and social enterprise sector groups. You will have great interpersonal skills in supporting people, community groups and local organisations.

This role is open to clinical staff and we welcome applications from nurses, mental health nurses and IAPT leads.
