About us:
In this role you'll work in one of our 13 Neighbourhood teams who operate from 07:00am - 22:00pm providing care for patients requiring intervention in their own home. The Neighbourhood Teams are part of a wider multi-disciplinary team to deliver 24/7 nursing care for patients in the community, which has recently rated 'Outstanding' for caring following CQC inspection. These unique integrated teams allow us to provide the right support, at the right time, delivered by the right people.

This is an exciting and varied role that incorporates many different aspects of patient care delivered to a wide range of patients. Plus the chance to develop and learn skills from team
members from a variety of professions.

About you:
You must have excellent communication, organisational and IT skills. You will need to assess and respond to the changing needs of the patient, working within your own limits and seeking support from the wider clinical team.

We'll support you with a full tailored induction programme and will encourage on-going development.

The contract is for a minimum of 30 hours up to a maximum of 37.5 hours (hours available can be discussed prior to application or at interview as required as a variety of hours are available).

Due to the nature of the role it is essential that you are able to travel independently across the city. If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.83 or Standard £30) from your salary. There will be no charge if you are enrolled in the DBS Update Service.
