Oxfordshire CCG believes that the introduction of Care and Support Planning (Year of Care) in general practice to be an essential step towards the delivery of integrated Diabetes care in Oxfordshire as part of the development of new models of care across the system in line with the NHS Long Term Plan. The introduction of the Year of Care concept will increase supported self-care and is focused on the priority area of diabetes in its first phase before application to all long term conditions.

 We are looking for a highly competent and self-motivated individual to join the Planned Care Team to oversee the delivery of our Care and Support Planning (Year of Care) programme. The purpose of the job is to assess, advise and support general practice teams to implement a YOC approach, ensuring participating practices deliver the intended service improvements with high quality care and outcomes. This will include the coordination of practice and locality based staff facilitating the implementation of Care and Support Planning.
