Have you ever wanted to work in the community?

Do you want the opportunity to empower patients and deliver complex care in their own homes?

If the answer is yes then this is the perfect time to join Leeds Community Healthcare!

Are you a Newly Qualified Nurse (Final Year Student)?

Community nursing suits people who are confident and enjoy working independently. You’ll be resilient, motivated and adaptable to working in an ever-changing environment. And of course, you have a passion for providing the best possible care. You must be a fully licensed driver with access to a vehicle suitable for business purposes.

Coming into a community workplace can be challenging for newly qualified nurses, so our preceptorship scheme is robust to match. Lasting up to 12 months, each NQN will have a dedicated preceptor. And this is built upon by our focus on clinical supervision, training and development, and access to a peer support network.

Are you a Qualified Staff Nurse?

We are looking for Registered Adult Nurses, with a passion for providing an outstandingcare. You'll be comfortable working both on your own and with a wide range of heathcare professionals. You'll be resilient, motivated and adaptable to working in an ever-changing environment. You must be a fully licensed driver with access to a vehicle suitable for business purposes.

In the community we offer a diverse range of holistic care which offers great scope for Staff Nurses to develop their clinical skills.

What's the job?

We are in the incredibly privileged position of caring for people in their own homes – generally the place they prefer to be treated, over a hospital or other medical setting.

Our integrated Neighbourhood Teams offer seamless support and care to our patient population. The teams include nursing, therapy and social care provision, and the focus at all times is on the individual – helping them stay at home, or to come home earlier from hospital. Our service runs 7am to 10pm every day of the year. The focus at all times is on the individual – supporting the patient to stay at home, admission avoidance and supporting our partners to facilitate hospital discharge.

Who are we?

Leeds Community Healthcare provides a range of community-based health services across the whole city. We treat people in their homes, local health centres and community hospitals, and work in partnership with other local Trusts, charities and Leeds City Council.

We have a strong culture based on deeply-held values, and our focus at all times is on delivering high quality care to the people of Leeds. In our most recent CQC evaluation, we were rated "Good", with our Adult Services rated "Outstanding" for caring.

What are the perks?

  • Competitive salary in line with Agenda for Change banding
