Moving forwards with an integrated workforce an exciting opportunity has arisen for an experienced, innovative Senior Nurse/ District Nurse who is looking for a new challenge.

This new way of working offers the opportunity to clinically help shape the development of the Neighbourhood Teams and contribute to key pathways and clinical frameworks, working across the health and social care economy.

About you
The successful candidate will be committed to improving patient care from the point of referral, as a Triage Clinician working in one of the thirteen Neighbourhood Teams this role requires the ability to utilise sound clinical judgement, high levels of communication and organisational skills alongside visible and effective leadership. There will be an expectation that some direct patient care and case management will be incorporated into the role.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.83 or Standard £30) from your salary. There will be no charge if you are enrolled in the DBS Update Service.
