An exciting opportunity has arisen working in the Welwyn and Hatfield locality as a caseload holder for the care homes in this area.

Recognising the issues facing care home residents and staff, the Welwyn and Hatfield (Welhat) Locality are seeking to recruit to a care home-based role The post will work autonomously and part of the core nursing team in conjunction with the Multi-disciplinary team. The applicant will be managed by the Deputy Locality manager within the Wel/Hat ICT.

The post holder will have experience of working in the Community Setting, liaising with GP’s, and other Multi-specialist Community Health Care workers including; Occupational Therapist, Physiotherapist, Community Nurses, Community Psychiatric Nurses and social workers.

You will work closely with a care home matron and pharmacist to assist in the delivery of the care home project, but will also cover other nursing homes within the Locality dependant on their needs.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,017 and a maximum payment of £1,762 per annum (pro rata where applicable).

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
