We have an exciting opportunity for a Complex case Manager to join the team working within the Beverley Locality. Part time hours available of 30hrs per week.

The successful applicant will need to be a registered nurse with either Specialist Practitioner Qualification, BSc District Nursing, Autonomous Practitioner/first contact Physical assessments skills or Degree level qualification in nursing clinical practice relevant to a complex case manager role. The post holder will be required to work as an autonomous practitioner, to manage a complex case load of patients requiring assessment, diagnosis and treatment, referral, review or discharge, utilizing a range of advanced clinical skills and expert knowledge for patients in their own homes and other community settings. To be able to navigate the patient through pathways across the primary, secondary interface, preventing unnecessary admissions, facilitating appropriate ones and expediting discharge home.

We are looking for highly motivated senior nurse who can demonstrate that they are committed to providing a high standard of care with the ability and skills to demonstrate strong clinical leadership and day to day managerial responsibility for the Community Nursing Team. Caseload management is key to this role, and you will be responsible for working closely with your Clinical Team Lead to manage the other members of the team, including staff nurses and health care support workers - to promote high quality nursing care.

The successful applicant will require excellent communication skills, be motivated and adaptable in their approach to care delivery to ensure that high quality care is maintained for the patient population.

Additionally, you will need to demonstrate IT literacy and this post requires that you have the ability to travel between locations to carry out your work and to attend training and meetings; daily access to transport is therefore essential.

The ability to commute within CHCP CIC's geographical areas with access to appropriate means of transport is essential.
