Gloucestershire Health and Care NHS Foundation Trust was formed on Tuesday 1st October 2019, following the merger of 2gether NHS Foundation Trust and Gloucestershire Care Services NHS Trust. The two Trusts joined together to provide integrated mental health, physical health and learning disability services to communities.

Senior Community Physiotherapist (part-time)

Band: 6

Salary: £30,401-£37,267 pro rata

Hours: 22.5hrs

Location: Dean House Cinderford, Gloucestershire

The Forest of Dean Integrated Community Team (ICT) is looking for a forward thinking, dynamic and motivated physiotherapist to work as part of the ICT, primarily to support the Complex Care at Home (CC@H) service. CC@H is a new and innovative, multi-disciplinary team, that focusses on the early identification of patients who are at risk of deteriorating health, wellbeing and/or frailty. The service works with patients and MDTs (including primary care) developing and delivering personalised care plans that support people to maintain their health and wellbeing, to prevent or delay crises. This includes promoting advanced care planning to enable appropriate responses should a crisis emerge.

The CC@H team is part of the ICT and consists of Community Matrons, Dementia Matron, Occupational Therapist, and Wellbeing Coordinators.

Your role will be to undertake assessments, reviews, case management and goal setting. This exciting position demands excellent team working and communication skills and an innovative and committed approach to providing high quality proactive patient care, within an integrated health and social service environment.

You will work alongside the ICT and CC@H and clinical supervision will be provided by the professional lead for physiotherapy and the wider MDT.
