We are looking for motivated, enthusiastic individual with appropriate experience to join a dynamic multi-disciplinary team within offender healthcare. CHCP CIC currently provides integrated offender healthcare services across HMP Hull and HMP Humber. We deliver a community-based model, bringing clinical expertise to the patients in our care, working closely with the multi-agency workforce.

 

The post holder will be responsible for developing and delivering public health programmes in line with Public Health England health promotion schedule to patients across offender healthcare service at HMP Hull with objective to reduce health inequalities, provide advice and expertise to facilitate healthy choices and support them to live healthy lives with continuity of care on return to the community.

 

Health promotion services work alongside primary care nursing, drugs and alcohol service and mental health teams custodial staff to specifically address:

  • Mental Health Promotion and Wellbeing

  • Substance misuse

  • Smoking cessation/reduction

  • Healthy eating and nutrition;

  • Healthy lifestyles, including relationships;

  • Sexual health 

  • Prevention of the transmission of blood borne viruses and engagement with blood borne testing

  • Support to patients to access to a range of physical exercise programmes appropriate to age and needs;

  • Healthcare support to rebuild fragmented family and peer relationships
