Pathway Navigator – Prostate Cancer

Job Reference: 412-S&C-3024

Mid Cheshire Hospitals NHS Foundation Trust
Urology Outpatients Department
£21,089 to £23,761 pa

We are looking to recruit an exceptional individual to join our Urology team to take on the new role of Pathway Navigator specifically for prostate cancer. We have received initial funding for this post from the Cheshire and Mersey Cancer Alliance as part of the national cancer strategy to implement rapid diagnostic pathways and appropriate risk stratified follow up pathways for patients requiring long-term follow up management after initial cancer treatment.

This role has been specially designed to support the proposed optimal pathway for prostate cancer and will require the post holder to evolve to support the pathway as it continues to refine over time. It is also expected though that the post holder will support all urological cancer patients throughout their journey from diagnosis to treatment and coordination of planned follow up appointments. Previous experience of working with cancer patients would be beneficial. This new permanent post will be based at Leighton Hospital though some travel may be required for patient events e.g. support group meetings and health promotion events.

The post holder will work both directly and indirectly with patients and their carers to assist in coordinating the care of patients through their hospital care journey, providing the patient with the relevant information and coordinate any test preparations that need to be sent or collected by the patient. You will be required to work across a range of IT systems to coordinate the booking of patient appointments.

It is expected that the post holder can work autonomously, using their own initiative and within a specialised team. The post holder is expected to be a highly experienced within the hospital setting with knowledge and evidence of specialised terminology, educated to a level required to perform the role or equivalent experience. Excellent organisational skills and the ability to confidently communicate with patients and a range of healthcare professionals is a key requisite for this role.

The successful applicant will be expected to provide administrative support and audio typing skills would be advantageous.

We will provide the necessary support to ensure your development within this role. If you are looking for a new challenge, enjoy variety in your work and wish to be part of the Urology Team striving to make a difference for our patients then we want to hear from you.
