An exciting opportunity has arisen for a nurse with a specialist interest in cancer to work as part of a new community cancer support team as part of the Leeds Cancer Programme.

This is a new service that will work based in the local community, but also work across organisational boundaries with secondary and primary care, community and voluntary sector colleagues in order to provide personalised support to cancer patients in their local area, throughout and beyond their cancer treatment.

The first team for this service is being developed within the Yeadon & Otley Primary Care Networks/Local Care Partnership, with potential to expand within 12-18 months. As such, you will need to work flexibly across the city according to the demands of the developing service.

The service will exist to support patients from the point of a cancer diagnosis, acting in a navigation role between all services, undertaking cancer care reviews and the delivery of health and wellbeing events in a community setting. This post will support the Clinical Team Lead nurse in developing close working relationships with all partners within the Local Care Partnerships, which includes health, adult social care, pharmacy and voluntary sector services, in order to ensure that the needs of the local populations are met.

About You

The post holder will be required to contribute to multi-disciplinary meetings (MDTs) and pathway development. We want to create a measurable improvement in cancer care, and as such the team will be required to collect audit and performance data to support the further development of this.

Applicants are expected to be registered nurses with experience of working in a primary, secondary or community setting with people with long term conditions and cancer. A post registration qualification or special interest in cancer would be desirable.

You will be able to work autonomously as well developing strong relationships with a wide range of different stakeholders the wider healthcare community, external organisations and the public to support pathway and service redesign.

The successful candidate will need to be self-motivated and have a flexible approach to work, with the ability to travel across Leeds as required.
