Band 6 Mental Health Nurse Posts within the Adult Mental Health Hospital Liaison Team (Adult Specialism)

Location - Woodlands Unit, Rotherham Foundation Hospital Trust Site

This is an exciting opportunity for a Registered Mental Health Nurse to join our innovative and existing Hospital Liaison Team which is expanding to meet the requirements of Core 24 and part of the urgent care pathway.

Your role will be working within the emergency and urgent care services as well as in-reach and case finding/treatment of people within the acute hospital wards. You will contribute to providing safe and supported discharge plans and facilitate alternatives to hospital admissions, including contributing to decisions on continuing health care, community care plans and safeguarding.

You will be a knowledgeable and a confident nursing practitioner with a proven track record in the care of adults with mental health problems with a willingness to continue to learn and develop within the role.

Your expertise will be valued in the clinical setting, but also in developing a new model of care. The team will work closely with colleagues in each specialism within the team, as well as a range of medical nursing and therapy staff in the acute hospital.

In supporting general hospital staff you will help develop positive working practices which put the patient at the centre of their care.

The service will involve a flexible duty rota covering seven days a week throughout a 24 hour period.
