The Wyvern Health Partnership will be a newly formed Partnership of 6 Practices in Swindon. The Partnership has 23 partners and a large team of GPs, Nurse Practitioners, General Nurses, Health Care Assistants and support staff, working together and sharing best practice to benefit patients.

All 6 are very busy practices with fantastic teams, which work well together, including training practices for GPs and nurses.

The new partnership aims to improve the quality of care to over 60,000 patients across the region. Each practice within the partnership is led by a group a partners, and a practice manager. The aim of the new partnership is to deliver a resilient, sustainable and efficient primary care service, which can attract and retain GPs, other clinical staff and support staff. This in turn will improve the quality of care, and patient satisfaction

Working with our existing Care Home Clinicians to support the work of the GPs in the practice with care-home visits and ward-rounds, with potential to include the management of home visits.

To deliver a high standard of patient care, using advanced autonomous clinical skills, and a broad in-depth theoretical knowledge base

To contribute to the QOF targets for the practice

Support the ongoing development of the Visiting service
