Community Link Worker

Community Link Worker Service – Wigan

Band 4 City Health Care Partnership CIC

Full-time: 37.5 hours per week

Do you want to work in a borough that puts people at the heart of everything we do?

We know that many people visiting their GP or hospital have underlying social problems that impact on their health. These could be related to a range of issues including social isolation, low level anxiety or stress, money or housing issues. Community Link Workers are based in GP Practices and help people to connect to the right support and services that can make a big difference to people’s lives.

 

A Community Link Worker:

  • Has lots of knowledge about local services and what is going on in the local community.

  • Will spend time to understand any issues or concerns that may be affecting health and wellbeing.

  • Will work with the client to find the right support and information.

  • Will share information about local services and activities that may be of interest.

  • Will provide practical support to access activities, services and local groups.

    You will need to be a strong team player with a ‘can do’ approach and a genuine interest in supporting people.

    Working as an integral part of GP Practices and Integrated Discharge Team as required, the role has a variety of functions requiring the successful applicant to demonstrate an innovative, pro-active approach.

    The post holder will possess excellent communication skills and be confident in the following:

  • The understanding of motivational approaches and coaching style conversations

  • The ability to build and nurture relationships at various levels

  • Supporting individuals to access a range of community activities and services dependant on individual needs and circumstances

  • Use of IT systems for administration tasks and up-dating client records.
