We are looking for an innovative, enthusiastic and motivated Band 5 Nurse to join Urgent Community Response working flexibly across the Exeter South West Locality, We are an integrated health and social care team who facilitates crisis management in the community, prevention of hospital admissions, and enable appropriate discharges, supporting people to remain as independent as possible at home. This is an exciting and fast paced team, where each day can present new challenges and learning opportunities. This post would be a fantastic opportunity for an experienced nurse, with a commitment to provide compassionate, holistic patient care, working flexibly across the area.

 

The Nursing role is pivitol within the Integrated Health & Social Care team linking with and supporting Primary care, Community Nursing and the Multidisciplinary Team. The use of clinical assessment skills and expert knowledge to make comprehensive and focused assessments, for patients presenting with a wide range of acute and long term conditions including frailty will be required. You will need to demonstrate flexibility, excellent time management skills and excellent communication and interpersonal skills. You must possess the ability to work autonomously with little direct supervision at times, with experience of lone working, whilst being a committed member of our supportive team.

The band 5 nurse will support the professional development of more junior staff focusing on individual competency and skill set and applies clinical leadership in the care of patients with the most complex needs. You will work with Managers/Senior Nurses/Clinical Leads to develop the service in line with patient’s needs and Trust wide developments. In return, supervision and professional development will be provided and supported, through dedicated mentorship and appraisal.
