In September 2017, we announced that our Trust Gloucestershire Care Services NHS Trust and 2Gether NHS Foundation Trust (2gether) were planning to join. From October 2019, we hope to be one organisation providing integrated community mental health, physical health and learning disabilities services. The new Trust will be called Gloucestershire Health and Care NHS Foundation Trust.

We are already working closely together but the benefits we really want for our communities can best be achieved if we become a single trust, delivering outstanding integrated physical and mental health care.

The aim of the proposed merger is to improve our services and ensure we are providing the most appropriate and seamless information, support and treatment for people with mental health issues, physical health problems or learning disabilities. It will mean we can help people as a ‘whole person’, not just separate conditions or symptoms. As healthcare professionals in the new Trust, you will be able to ensure better outcomes for service users which we know is really important to all clinical, medical and support professionals alike. For colleagues, it will also provide more opportunity to develop and realise your potential.

While our overall name will become Gloucestershire Health and Care NHS Foundation Trust, services in Herefordshire will be given their own, separate, name, which will be Herefordshire Mental Health and Learning Disability Services.

The details of the merger will be finalised early September and the Trusts hope to be one organisation from 1st October 2019.

Both Trusts are high performing with ‘good’ CQC ratings and strong financial foundations.


Nurse Advisor, Care Home Support Team

Band: 5

Salary: £24,214-£30,112

Hours: 18.75 hours available

The Gloucestershire Care Services Care Home Support Team is a multi-disciplinary team made up of registered nurses, an occupational therapist and a dietitian. The extended team also consists of a designated CCG pharmacist and registered mental health nurses of 2gether NHS Foundation Trust. The team all work together with managers and staff of residential and nursing homes across the county to build relationships, promote best practice in care, and provide training in order to support providers to deliver safe high quality care.

This highly respected and effective team has done much to support the care home community in their continual drive to raise standards of care and to act as a link between care homes and other stakeholders in the wider health and social care arena in Gloucestershire.

An exciting opportunity has arisen for an enthusiastic, self-motivated individual to join the Trusts’ Care Home Support Team. As a Nurse Advisor you will be required to contribute to delivering the following objectives:

  1. Work with the specific aim to gain an understanding through case review of avoidable admissions from Residential Care Homes into Hospitals. Use current research to increase knowledge of avoidable admissions from residential care homes into hospitals.
  2. .Developing the skills/knowledge of care home care-assistants and nurses through training, coaching and case management, supporting them to embed training in practice.
  3. Working in partnership with care homes and discharge teams based in the acute hospitals to facilitate timely and safe transfer of patients between hospital and care home settings; strengthening communication channels to support proactive person centred discharge plans.

The Registered Nurses in the Care Home Support Team also provide a physical health liaison service for patients residing in Charlton Lane Hospital who have complex mental health conditions.

The successful candidate must have experience of working with older people and experience of caring for people who have Dementia/Mental illness

We are looking for Nurses who are dedicated to delivering and promoting the highest levels of quality person centred care. Good interpersonal/ communication skills and the ability to work well in a team are essential, together with the aptitude to work autonomously using your own initiative.
