There are 6 Clinical Commissioning Groups (CCGs) in South West London (SWL), Croydon, Kingston, Merton, Richmond, Sutton and Wandsworth, working collaboratively to contribute towards the SWL Medicines Optimisation work plan. Merton, Sutton, Wandsworth, Kingston and Richmond CCGs form part of the SWL Alliance, with Merton and Wandsworth, and Kingston and Richmond under a single management structure under the Accountable Officer for the Alliance. The total primary care prescribing budget across SWL CCGs is circa £172m.

SWL has developed a 5 year Sustainability and Transformation plan (STP), with NHS organisations and local authorities coming together to develop ‘place-based plans’ for the future of health and care services for the future of health and care services in the area. The scope of STPs is broad covering three headline issues: improving quality and developing new models of care; improving health and wellbeing; and improving efficiency of services. The SWL Medicines Optimisation work plan forms part of the SWL STP under the oversight of the SWL Transformation Office.

The post holder will be hosted by Wandsworth CCG, enabling them to integrate into a CCG medicines optimisation team and facilitate working across all CCGs in SWL.

A novel opportunity has arisen across South West London (SWL) within the Medicines Optimisation Workstream for an enthusiastic and highly motivated dietitian to provide strategic leadership in the area of improving appropriate prescribing of nutrition products in primary care. The post holder will be leading this project at an exciting point in shaping the delivery of high quality, safe and cost-effective prescribing of nutrition products that meet our patients nutrition and hydration needs. The post holder will provide professional and clinical leadership to a small team of primary care dietitians who are working in CCGs medicines Optimisation teams in SWL.

The post holder must be a registered dietitian who is an excellent communicator with good interpersonal and relationship building skills and has a wide range of dietetic experience including primary care.
