CSH Surrey is Surrey’s largest and longest established NHS community services provider (since 2006). We are part of the NHS, so our 1500+ employees get NHS pay and pensions, and also receive the Fringe ‘High Cost Allowance’ of 5%.

Our nurses, therapists and support staff provide adult community health services in the mid and north west Surrey areas, and children’s community health services for the whole of the county. Together we serve a population of more than 1 million people.

In June 2017 our services received an overall ‘Good’ rating from the CQC, with ‘Outstanding’ for how caring our teams are.

We provide excellent training and development opportunities, including the care certificate, apprenticeships, numeracy and literacy courses, access to the Nursing Associate programme, and a wide variety of management and leadership courses and programmes. We also offer a supportive preceptorship programme for newly qualified nurses and a well-established rotational programme for therapists. Our nurses and therapists also benefit from access to bespoke clinical skills courses.

Surrey Downs Health & Care

As a partnership between the three GP Federations, community trust and acute trust, Surrey Downs Health and Care is delivering true transformational change for adult community services across Surrey Downs to the benefit of patients and the local healthcare system. Our delivery model is formed around a shared vision and innovative use of the wealth and diversity of both the clinical and managerial experience that the partnership brings and as such it will provide a truly holistic, person-centred service that no one partner could provide on its own.

The model focusses on care being provided through distributed clinical leadership using Primary Care Networks (networks of GP practices working together with combined practice populations of around 50,000); integrated community bedded services and specialist services with the development of pathways into and out of the acute trust.

Do you want to be part of an experienced, supportive and integrated team that provides outstanding rehabilitation in the community? Do you want a development role that provides a fulfilling and exciting mix of long term rehab work and fast-paced rapid response work? And would you be proud to work for a team that regularly and consistently receives really positive feedback from patients and family members? If the answer is ‘yes’, you should join us! Our Integrated Rehabilitation Service is looking to recruit a permanent Band 3 Rehabilitation Assistant to join our friendly and dynamic team in a full time role. The service currently operates daily between the hours of 8am-6pm, covered by 2 shifts, and the post will include weekend working on a rota basis. You will be based within one of the Primary Care Networks (Dorking, Epsom or Molesey) but working throughout the Surrey Downs Health and Care area.

The service works closely with the Community Hubs and the Adult Social Care teams in order to provide an integrated approach that puts the patient at the heart of the service. This rewarding role involves carrying out equipment and walking aid assessments autonomously and delivering rehabilitation programmes to patients in their own homes with guidance and support from Physiotherapists. The role is community-based so it is essential that you are a car driver and have access to a car for work. As well as an extensive training programme within Surrey Downs Health and Care, our team also has an excellent in-service training and clinical supervision programme. In addition, funding is available for external courses, with current staff having attended events such as the Virtual Dementia Tour. Please see the job description for full details. Please contact Heather Taylor, Integrated Rehabilitation Service Team Leader, on 01372 734791, if you would like to discuss the role.

While this post remains within CSH, the post will be seconded to Elmbridge Federation. This will not impact on any of your terms of employment.
