As a community nurse   delivering NHS services with us you will receive  AFC band  5 salary  (23,023-29,608 ) and terms and conditions  including an NHS pension. We’ll provide you with access to a committed Learning and Development team, and we work closely with Universities and Health Education England to further your potential and support your progression. We will offer a combination of in house eLearning, workshops and courses to support your development.

You  will have a RGN  qualification. As   a community nurse   You will visit   housebound patients in their own homes to undertake   holistic assessments.

You will be responsible for contributing to the holistic assessment, planning delivery and evaluation of patient care. You will implement, monitor and maintain high standards of care. You will also be required to use electronic patient record systems, be able to prioritise your work load

If you are able to demonstrate excellent communication, clinical, initiative and time management skills; are motivated, innovative and flexible .we would invite your application  for our consideration

As a community nurse  you will be working closely with our colleagues in Virgin care as well as with the wider Primary Care Team providing care to people in their own homes for a range of long term conditions. Our work is crucial to the community as we aim to prevent hospital admissions for our patients. You will care for patients and their families in the way you believe is best, delivering our high service standards, sharing best practice and actively improving the way we work. You will be based primarily in   Keynsham  Health Centre.

If successful you will be joining a friendly   supportive team who actively encourage suggestions for quality improvements.
