Community Occupational Therapist, Band 6


The Greenwich Community Rehabilitation and Short Term Assessment team is seeking an enthusiastic occupational therapist to join our team. The team comprises of Occupational therapists, Physiotherapists, nurses, social workers, Care managers, assessment officers and therapy assistants. The service is well established and was developed to work with adults within their home, to maintain independence and improve the quality of life with a view to promoting healthy and active aging.


You will provide occupational therapy input to clients referred to the Community Rehabilitation and Short term Assessment Team. This will include providing an assessment and provision of both non-complex and complex equipment to ensure that the client is safe and to maintain independence in their own home. The post holder will be required to work in a multidisciplinary way and work jointly with the other team members to ensure that all care needs are met. They will be required to carry out an holistic assessment of the client within their own homes and devise and contribute to a multidisciplinary care plan. This will include making onward on referrals to other agencies as appropriate.


You will need to be a versatile, adaptable team player who has good organisational skills. You will be required to use multiple computer systems therefore a good knowledge of IT is essential.
