Gloucestershire Health and Care NHS Foundation Trust was formed on Tuesday 1st October 2019, following the merger of 2gether NHS Foundation Trust and Gloucestershire Care Services NHS Trust. The two Trusts joined together to provide integrated mental health, physical health and learning disability services to communities.


OCCUPATIONAL THERAPIST

Band: 6

Salary: £30,401 - £37,267 pro rata

Hours: Various options regarding hours are available, including:

  • 37.5 hours per week for 33 weeks
  • 22.5 hours per week for 1 year

We will consider a working pattern between 22.5 and 37.5 hours per week with the length of contract being on a pro-rata basis

Location: Cotswolds Integrated Community Teams – South Cotswolds

Due to Maternity leave and an internal secondment, we have an opportunity for an experienced Occupational Therapist to join our Integrated Community Teams (ICTs) based in Cirencester. The ICT consists of Community Nursing, Physiotherapy, Occupational Therapy and Reablement who work closely with our co-located Adult Social Care colleagues.

We work in partnership with GPs, community hospitals, mental health services and voluntary organisations to enable people to live as independently as possible at home. The ethos of the teams is a Reablement, person-led approach to recovery and self-management, to ensure we offer care close to home, thus avoiding unnecessary admissions to bed based services.

We are an active player in the development of the local Primary Care Network entering an exciting time as we work with our partners to develop local place based models of care and merge organisations with the local Mental Health and Learning Disability Trust, 2Gether NHS Foundation Trust.

It is a multifaceted role involving both triage and urgent response for rapid assessment of patient need, as well as a caseload of patients in one of the services we provide: bed based Reablement, community Reablement or long term condition management. This is an ideal opportunity for a Band 6 therapist wanting the opportunity to work in a community team at the forefront of development across the urban and rural communities of Gloucestershire.
