Chesterfield and Dronfield Primary Care Network (PCN)

Arc Primary Care is an umbrella organisation which represents 12 GP practices in Chesterfield and Dronfield (Chesterfield and Dronfield Primary Care Network). The PCN is committed to improving the care of patients in Chesterfield and Dronfield and sustaining General Practice.

 

We are fortunate to be working with Derbyshire Voluntary action to take forward Social Prescribing in this area. Social prescribing empowers people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.

 

If you are a self-motivated, dedicated ‘people-person’ whose passion is to support and empower individuals to take control of their health and wellbeing, you could be just what we are looking for.

 

If you are a creative, innovative thinker, who would love the challenge of working across exciting new partnerships within the Primary Care Network and working collaboratively with NHS and Voluntary Sector colleagues in Chesterfield, this could be the job for you.

 

There are two posts, both will be employed by Arc Primary Care and have access to the NHS Pension scheme, but will be part of the team of Link Workers at Derbyshire Voluntary Action’s ‘Connect to…’ social prescribing service. You will work closely with the GP practice teams and the Clinical and Management leads for the project.

 

You will adopt an holistic, non-medical, person-centred approach to your caseload, co-designing action plans to improve health and wellbeing based on ‘what matters’ to the person and their individual strengths and needs – and supporting them to achieve their goals.

 

Ideally you will have worked in a similar role before, but this position would also be suited to someone with good local knowledge, appropriate experience and enthusiasm. If you are proactive, enthusiastic, person-centred and hold a clean driving licence with unlimited access to a car, then we would love to hear from you.
