We are recruiting for a number of new positions with flexible working patterns to help support new ways of working within health and care in Leeds.

Local Care Partnerships
Leeds is supporting a model of joined up working (Local Care Partnerships - LCPs) to deliver local care for local people, working in and with local communities to improve health and well-being All LCP's share the same key feature -a range of people working together, regardless of the employing organisation, to deliver joined up collaborative care that meets the needs of each local population. LCPs build the strengths and extensive history of col laboration in the city to realise some of the most exciting, person centred transformation in health and care.
LCPs are in development and require a range of support. You will be part of a new focused team from different backgrounds across health, social care, third sector and academia to support this important work. The team will provide central capacity to accelerate transformation and change across all LCPs. The LCP development team will be supporting multiple projects and partnerships to form. You will have a key role to play in helping to shape a new culture to support change across Leeds that puts people at the centre of their care.

Population Health Management
Leeds wants to be a healthy and caring city for all ages, where people who are the poorest improve their health the fastest. We believe that using a Population Health Management (PHM) approach will be a key mechanism to enable us to achieve this commitment and have already used this approac h to improve outcomes for people living with frailty in Leeds
We are about to begin the next part of the Leeds Population health management journey working with 'Wave 2' of our Local Care Partnerships and would like to recruit a team of Project Support Officers to join us to help this exciting roll out and support implementation and evaluation of specific initiatives

What are we looking for?
We are looking for motivated people who will be responsible for ensuring the delivery of an effective administration and support service to these programmes. You will prioritise and manage your own workload independently but will react to a varied and changing work programme. You will be responsible for supporting change projects around care re-design, working in an environment of multiple partners and views. Part of the role will be to work with teams to show progress and offer support to maintain momentum. The role will require you to be hands on and flexible, working with a wide range of stakeholders. It is people that make the power for positive change, and we need the right people here in our team.
For this role, you will need excellent communication and people skills, the ability to work well with others and develop good relationships.

You will also have a strong background an d skills in one or more of the following:
. Project Management Support
. Senior Administration
. Communications

You will gain insight into new ways of working across health and care, to further develop your existing skills and knowledge, be valued and be part of a supportive friendly and highly motivated team.
