Urgent Care Coordinator

The coordinator role will include taking urgent care referrals from GPs and other Health and Social Care professionals, rostering theRapid Response Support Workervisits and passing all referral information to the appropriate Urgent Community Response Team. The role will also be the point of contact for the Rapid Response Support Workers up to 22.30hrs

The working hours for this role are 07.00hrs to 22.30hrs across 7 days a week. The roster will be a mixture of early, mid and late shifts.

We are looking for a motivated member of staff to work as part of the team across 7 days to ensure flexibility and provide cover for each day. If you can demonstrate your flexibility within your working pattern, commitment and willingness to be part of a hardworking and friendly team then this could be the opening for you.
