An exciting opportunity has arisen for a Diabetes Specialist Nurse .We are looking for enthusiastic experienced Diabetes Specialist Nurse to join our Diabetes Specialist Nursing Team at CHFT. This role will also include working closely with the community and also providing the level 4 and level 5 services including in-patient care. This person must be passionate about working together with people with diabetes and their families. It would be of benefit to have an interest in ante natal, adolescents and insulin pump therapy. You ideally would be an independent Prescriber or willing to work towards it. It is essential to have experience on the initiation of injectable therapies. You will work closely with the all members of the Diabetes Specialist Nurse team and the wider multidisciplinary diabetes care team, in the support, education and management of people with diabetes

As an autonomous practitioner you will be responsible for managing your own caseload of complex patients. You will have a full clinical caseload of referred patients with whom you will work autonomously and be solely responsible for advanced assessment, planning, implementation and treatment. Through effective working practices you will continue the philosophy of jointly working with other health care professionals to develop a holistic support network for people with diabetes within the Trust, Specialised Units and primary care.

You will be expected to contribute to the organisation and delivery of ongoing diabetes education and training for other health care professionals and carers in diabetes, and you will also be expected to participate in the development and instigation of local diabetes care strategies and guidelines.

Good communication and time management skills are essential, including the ability to work with a diverse group of people.
