As part of our integrated Community Care Bed team, you'll offer seamless support and care to our patient population, alongside other professionals in nursing, therapy and social care provision. Our service runs 24 hrs every day of the year, flexible shift patterns will be considered. We have 72 community care beds across our partnership and the bases are in the south of the city, LS11 and the North West, LS19. The focus at all times is on the individual - helping them to progress to home.

We are in the exciting and forward thinking position of being in a partnership service with Leeds city council and provide temporary care and rehabilitation for the most vulnerable older patients living with frailty, not quite well enough to be in their own home but not in need of hospital care.

About You

You're a registered adult nurse, with a passion for providing an amazing level of care. You'll be comfortable working both on your own and with a wide range of other people. You'll need to be resilient, motivated and adaptable to working in an ever-changing environment.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £48.83 or Standard £30) from your salary. There will be no charge if you are enrolled in the DBS Update Service.
