An opportunity has arisen for a highly skilled and enthusiastic clinician to join our team. The successful candidate will be keen to join the well-established leadership team and be able to use their extensive experience to provide and promote the delivery of compassionate community health care. The post holder will support the teams with clinical caseload management while supervising and supporting the professional development of Senior Community Nurses, Community Nurses, physiotherapists, Occupational therapists, Healthcare Assistants and also students We expect to employ a person with person centred values and a motivated clinician who will be excited by the opportunity to help us provide and teach others to deliver the experience of care you would wish to receive yourself.

The teams consist of Community Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistants and administration staff with GP and social care colleagues working closely alongside.

The teams deliver planned nursing, therapy and admission avoidance / supported hospital discharge interventions to the adult patient population. The teams remit is to deliver a high quality service to people in their own homes, putting them at the centre of care and utilising the skills of all the multidisciplinary team where appropriate.

 

We are looking for a highly motivated, flexible team player with a strong, quality driven patient focus and an ability to work through change to embed new ways of delivering care. You should have excellent clinical, written and verbal communication skills.

The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
