A unique opportunity has arisen within Continuing health care and The Personal Health budgets team.

We are looking for a dynamic individual to join our team ofcommunity based clinicians and Administration staff within ContinuingHealthcare and Personal health budgets.

A team approach is implemented shared development opportunities for staff dealing with complex care for patients with identified primary health care needs.

We require an enthusiastic, motivated and dedicated individual to join our existing welcoming and nurturing team.

We are looking for an individual who can liaise and work well as part of an MDT and with stakeholders .

The preferred candidate will have the ability to work collaboratively with the local authority in the implementation of joint processes.

We are looking for an enthusiastic and reliable person to work as part of our service area.

We require an individual who hold excellent Core assessment skills and holistic evidence based patient focused practice values.

The role is full time and requires applicants to hold a driving licence and Registered Nursing qualification extended to holders of Nursing registration within Learning disability and Mental health specialities.

Internal preceptorship into the exciting world of Personalisation and continuing health care will be given with a robust programme of induction.

Applicants should hve a good understanding of MCA and awareness of the DOH Framework for CHC continuing health care Transition and personalisation principles.
