We are changing the way we deliver our adult community services across east and north Hertfordshire to create a single, integrated model of care. Our aim is to make healthcare more preventative rather than reactive, and to empower local teams to tailor services to the needs of their local communities.

Commissioned by East and North Hertfordshire CCG, the service is provided by Hertfordshire Community NHS Trust, Hertfordshire Partnership University Foundation Trust and Hertfordshire County Council’s Adult Care Services. The partnership organisations are working collaboratively across nursing, therapy, mental health and social care to deliver better care for our population

Exciting opportunities have arisen in our Community Integrated Care Teams for Nurses, which delivers Prevention of Admission, Case Management and Core Nursing and Therapy to residents of Upper Lea Valley (Hoddesdon and surrounding areas)

The Assistant Practitioner will demonstrate a high level of adult community care skills and competency when undertaking care for adults. The service runs from 8am until 10pm, 7 days a week, working a shift system.

The range of identified skills will be determined by the Team Manager to reflect the needs of patients with complex health needs under the care of clinical staff. The post holder will provide direct supervision to Health Care Assistants and will be supported in this role by Team Leaders and other clinical staff in the team.

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

In addition to the basic salary, posts attract a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,017 and a maximum payment of £1,762 per annum.
