We are seeking to appoint a Band 6 Mental Health Practitioner to work within the Great Yarmouth & Waveney Crisis Resolution & Home Treatment Team (CRHT) based at Northgate Hospital Acute Services. The team is unique in the way that it integrates both Inpatient and CRHT care. We work closely with the Ward as patients discharged follow a planned pathway of care and receive both inpatient and post-discharge CRHT treatment, which offers ongoing risk management and after-care involvement within the Community.

The role will include carrying out emergency assessments from Community Teams, GPs and A&E Out of Hours in view of gate-keeping possible admissions, ongoing care or referral to external team. The team also visits Service Users at home to plan and implement care packages for Crisis and Home Treatment Team patients.

You will have excellent mental health assessment skills, time management skills, risk management skill and will need to be adaptable and flexible within their role.

Previous experience within an Acute setting and risk management would be beneficial/advantageous.

You will be able to work flexibly as this service covers 24 hours 7 days a week. Although you will be working as part of a multi-disciplinary team, there will be elements of lone working. The post will sit under the Great Yarmouth & Waveney Acute Mental Health Services within a supportive and progressive Locality.

This update service is an annual subscription at a cost of £13 to you. This will enable the Trust to have instant online access to your DBS record, with your consent, and so will remove the need for you to have to apply for this repeat check again.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
