The Practice Based Medicines Optimisation Team has recently won the Primary Care Pharmacist Association 2019 conference Poster Presentation. The team have been established for 2 and half years, working in General Practice across North Manchester providing a clinical pharmacy service.

The introduction of Primary Care Networks (PCNs) has presented a unique opportunity for the expansion of the practice based medicines optimisation team across the city into the majority of Manchester PCNs.

The Medicines Optimisation Team has a long standing history of delivering high quality services to support the work of General Practice across the City. The successful candidates will be welcomed into the expanding medicine optimisation team with the unique opportunity to develop both clinical and strategic skills.

We are seeking innovative pharmacists to provide clinical pharmacist services across Manchester. With strong clinical leadership and established links with General Practitioner trainers, we welcome applications from pharmacists working in all sectors who are looking to develop their role within General Practice.

Successful candidates will be involved in the delivery and evolution of clinical pharmacy services within general practice and will demonstrate a passion for providing patient centred care. In return, they will be supported with their training and developmental needs by providing:

  • one to one mentorship with a dedicated GP during induction phase

  • regular team meetings focussing on group based learning to support clinical knowledge

  • a senior management team with a strong clinical background

  • peer mentorship from the existing team

  • access to non-medical prescribing course

As a GP practice based pharmacist successful candidates will be responsible for providing a clinical pharmacy service to up to 3 GP practices in Manchester. This will involve working in the practices day to day and engaging with the practice staff and becoming an integral part of their team. The successful candidates will see patients face to face, supporting them with their medication needs; review discharge prescriptions and support practice staff with medication related queries.
