In September 2017, we announced that our Trust Gloucestershire Care Services NHS Trust and 2Gether NHS Foundation Trust (2gether) were planning to join. From October 2019, we hope to be one organisation providing integrated community mental health, physical health and learning disabilities services. The new Trust will be called Gloucestershire Health and Care NHS Foundation Trust.

We are already working closely together but the benefits we really want for our communities can best be achieved if we become a single trust, delivering outstanding integrated physical and mental health care.

The aim of the proposed merger is to improve our services and ensure we are providing the most appropriate and seamless information, support and treatment for people with mental health issues, physical health problems or learning disabilities. It will mean we can help people as a ‘whole person’, not just separate conditions or symptoms. As healthcare professionals in the new Trust, you will be able to ensure better outcomes for service users which we know is really important to all clinical, medical and support professionals alike. For colleagues, it will also provide more opportunity to develop and realise your potential.

While our overall name will become Gloucestershire Health and Care NHS Foundation Trust, services in Herefordshire will be given their own, separate, name, which will be Herefordshire Mental Health and Learning Disability Services.

The details of the merger will be finalised early September and the Trusts hope to be one organisation from 1st October 2019.

Both Trusts are high performing with ‘good’ CQC ratings and strong financial foundations.

Community Physiotherapist

Band: 5

Hours: 22.5 hours

Salary: £24,214-£30,112 pro rata per annum

Base: Stroud Locality; Redwood House, Stroud

We are looking for an enthusiastic physiotherapist with appropriate knowledge and skills to work within our well established Integrated Community Team (ICT) working in the Stroud locality.

This role is an exciting opportunity working as a band 5 Physiotherapist, in which you will manage a wide caseload including neurological rehabilitation, falls assessment and prevention and long term conditions, in a fully integrated MDT setting. Responsibilities include assisting in preventing hospital admission, facilitating appropriate and timely hospital discharges as well as delivering rehabilitation programmes to people within their own homes and working alongside the reablement team. You will also assist in the supervision of students and physiotherapy assistants.

The successful candidate will take an active role within the operational and clinical running of the Active Balance Group in a community hospital outpatient setting. This will include attending countywide falls forum meetings to share best practise and service development alongside Falls Clinical Specialist Physiotherapy for clinical support in practise.
