Mid Cheshire Hospitals NHS Foundation Trust manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich and Elmhurst Intermediate Care Centre in Winsford. We are the Lead Employer for Central Cheshire Integrated Care Partnership (CCICP) which aims to transform, develop and deliver health care services in the community.

This is a brilliant opportunity to join our forward thinking Care Community Teams in Winsford, Crewe and Natwich as a Complex Case Practitioner.

Supporting patients in their own homes with complex health and social care needs you will undertake holistic physical assessments and provide appropriate intervention whilst working in collaboration with other internal and external stakeholders.

As developing Care Communities in Winsford and Crewe, we are proud of the transformation work we have already achieved in order to establish the “Right Professional, Right Place, Right Time” approach. As a result of this you will empower patients and promote independence whilst avoiding unnecessary hospital admissions, in addition you will also provide elements of support for patients who require on going assessment following discharge from the acute sector.

 

With a background in health and social care you will have a broad clinical skill set with application to patient centred leadership with elements of managerial duties. In addition you should be able to work as part of a dynamic and diverse team as well as autonomously furthermore you must be knowledgeable and have a sound understanding of long term condition management.

You will be required to maintain a punctual and timely approach to your work as well as the need to work flexibly within your role. Professionalism is also a key quality to uphold the reputation of CCICP and the Care Communities that provide excellent healthcare to our patients.

You will also have an allocated mentor to work alongside you during the induction and probation process. You will also have the benefit of working within an integrated nursing and therapy team, who look to support and develop staff to constantly improve levels of patient care.

In September 2016 Central Cheshire Integrated Care Partnership (CCICP) came into being as an innovative health partnership between Mid Cheshire Hospitals NHS Foundation Trust, Cheshire and Wirral Partnership and South Cheshire and Vale Royal GP Alliance, contracted to provide community health services for people across South Cheshire and Vale Royal. The aim of the partnership is to innovate, transform, develop and deliver services located within the community.
