In September 2017, we announced that our Trust Gloucestershire Care Services NHS Trust and 2Gether NHS Foundation Trust (2gether) were planning to join. From October 2019, we hope to be one organisation providing integrated community mental health, physical health and learning disabilities services. The new Trust will be called Gloucestershire Health and Care NHS Foundation Trust.

We are already working closely together but the benefits we really want for our communities can best be achieved if we become a single trust, delivering outstanding integrated physical and mental health care.

The aim of the proposed merger is to improve our services and ensure we are providing the most appropriate and seamless information, support and treatment for people with mental health issues, physical health problems or learning disabilities. It will mean we can help people as a ‘whole person’, not just separate conditions or symptoms. As healthcare professionals in the new Trust, you will be able to ensure better outcomes for service users which we know is really important to all clinical, medical and support professionals alike. For colleagues, it will also provide more opportunity to develop and realise your potential.

While our overall name will become Gloucestershire Health and Care NHS Foundation Trust, services in Herefordshire will be given their own, separate, name, which will be Herefordshire Mental Health and Learning Disability Services.

The details of the merger will be finalised early September and the Trusts hope to be one organisation from 1st October 2019.

Both Trusts are high performing with ‘good’ CQC ratings and strong financial foundations.


Community Stroke Co-ordinator

Band 6

Hours available: 48.75

1 x 22.5 hours per week, permanent contract, immediate start

1 x 26.25 hours per week, fixed term contract to cover maternity leave October 2019 - September 2020

Base: Independent Living Centre in Cheltenham – with a requirement to work across the county.

These exciting posts based in Cheltenham with the Stroke Early Supported Discharge team have become available due to retirement and imminent maternity leave.

This role involves visiting stroke patients in their own homes and providing information and support to patients and their carers in accordance with the RCP and NICE guidelines. There is close liaison with the Acute Trust and support from the other specialist stroke community nurses in the team.

Experience in stroke care and multidisciplinary working are essential as well as excellent communication and problem solving skills. The successful candidates will be autonomous practitioners with good self-management ability.
