Central Cheshire Integrated Care Partnership (CCICP) brings together three organisations that aim to transform, develop and deliver community health care services that are focussed on delivering high quality, safe care in the right place at the right time.

CCICP is a unique collaboration between Mid Cheshire Hospitals NHS Foundation Trust (MCHFT), Cheshire and Wirral Partnership NHS Foundation Trust (CWP), and the South Cheshire and Vale Royal GP Alliance, which covers 29 local GP practices. More recently, CCICP has aligned its services into different geographical locations creating Care Community hubs. These have become a central point of contact and allow referrals to be made in a more efficient way.

Reporting to the Transformation Programme Manager, you will be working closely with stakeholders in CCICP, Mental Health, Social Care partners and the voluntary sector to deliver our exciting Frailty project. The project is designed to further improve care and support for individuals living with frailty in our five Care Communities across South Cheshire and Vale Royal.

This six month project has four deliverables:

  • To strengthen and enhance existing community resources to build on our existing teams and improve support and care for our frail patients.
  • To deliver ‘Independent Living Clubs’ across our five Care Communities which will bring together health and social care professionals and the voluntary sector into one location to support our patients.
  • Implement Community Frailty Clinics for patients who require more intense support, access to senior clinical input or diagnostic evaluation.
  • Embed risk stratification into our five Care Communities to help us know our patients better and respond to their needs earlier.

To succeed in this role you will have proven design and project management experience with the health and social care environment and you will be used to working with senior managers, clinicians and stakeholders across the health economy to deliver the frailty project outcomes and benefits.

You will be used to preparing and presenting reports to a variety of audiences and use your well-developed negotiating and influencing skills as the project requires.
