We believe that the introduction of Local Neighbourhood Teams provides the essential foundation upon which to build and develop out of hospital care and improve patient experience – with teams built around GP practices. Our vision is to secure Local Neighbourhood Teams with ‘one team identity’ working to an agreed set of goals and priorities regardless of who employs different staff. This will improve coordination of care and communication and reduce duplication. We aim to deliver an effective, high quality service that puts patients at the centre of their own care.

Join us in the early stages of our journey within Worcester City where we have set up 2 Neighbourhood teams – Riverside and Cityside.

This is an exciting opportunity for an experienced, flexible and dynamic RGN, RMN, Occupational Therapist or Physiotherapist to lead Cityside Neighbourhood team. Although 12 months into our Neighbourhood journey, we believe that the timing will still give the successful applicant the opportunity to shape the team and future service.

You will be responsible for the operational management and clinical leadership of approximately 27 Nurses and 6 Health Care Assistants who deliver a range of community assessments and treatments. You will have line management responsibility for 9 Senior Nurses and will be expected to deliver regular 1:1s and team meetings to support staff and ensure excellent communication within the service. You will be expected to have oversight of the caseload, using IT reporting systems to support you as well as attending the City’s complex MDT meeting, where you will become involved as appropriate with complex cases.

You will be supported by the Integrated Community Service Lead and work closely as a team with four fellow B7s who include the Single Point of Access Lead, Clinical Lead Physiotherapist, Clinical Lead Nurse and the Riverside Team Leader. You will also work collaboratively with colleagues from Primary Care, Social Care and the Acute Trust to achieve the best outcomes for patients and to maintain flow within the care system, bringing together planned and unplanned services to deliver seamless care and reduce emergency hospital admissions. Where appropriate, our philosophy is to always maximise independence and the post-holder will be expected to promote the role of multi-disciplinary working, utilising our therapists and HCAs to re-able and rehabilitate our patients, reducing the need for ongoing care where appropriate.
