In September 2017, we announced that our Trust Gloucestershire Care Services NHS Trust and 2Gether NHS Foundation Trust (2gether) were planning to join. From October 2019, we hope to be one organisation providing integrated community mental health, physical health and learning disabilities services. The new Trust will be called Gloucestershire Health and Care NHS Foundation Trust.

We are already working closely together but the benefits we really want for our communities can best be achieved if we become a single trust, delivering outstanding integrated physical and mental health care.

The aim of the proposed merger is to improve our services and ensure we are providing the most appropriate and seamless information, support and treatment for people with mental health issues, physical health problems or learning disabilities. It will mean we can help people as a ‘whole person’, not just separate conditions or symptoms. As healthcare professionals in the new Trust, you will be able to ensure better outcomes for service users which we know is really important to all clinical, medical and support professionals alike. For colleagues, it will also provide more opportunity to develop and realise your potential.

While our overall name will become Gloucestershire Health and Care NHS Foundation Trust, services in Herefordshire will be given their own, separate, name, which will be Herefordshire Mental Health and Learning Disability Services.

The details of the merger will be finalised early September and the Trusts hope to be one organisation from 1st October 2019.

Both Trusts are high performing with ‘good’ CQC ratings and strong financial foundations.


Position: Physiotherapist – Rotational

Pay Band: 6

Hours: 37.5 Hours per week (flexible working requests will be considered)

Base: Cheltenham, Independent Living Centre

An exciting opportunity has arisen to recruit a dedicated and motivated Physiotherapist on a substantive contract to become part of a dynamic and established Integrated Community Team (ICT) within Cheltenham.

Gloucestershire Care Services deliver community health & social care services for adults & older people via a model of locality based cluster teams. Each of the clusters consist of nursing, therapy and reablement teams working closely alongside social work professionals to provide support to people within their own homes and within bed based reablement units. We work closely with GPs, local communities, acute and community hospital(s) and other partners to enable people to live as independently and for as long as possible within their own home.

We are seeking an experienced physiotherapist to join the team as part of a planned series of rotational posts (9-12 month duration) starting within a core ICT role and rotating to other community areas such as priority response work via the referral centre, bed based reablement and Complex Care at Home (CC@H) all based within Cheltenham.

The CC@H is a new and innovative multi-disciplinary team, that focusses on the early identification of patients who are at risk of deteriorating health, wellbeing and/or frailty. The service works with patients and multidisciplinary teams (including primary care) developing and delivering personalised care plans that support people to maintain their health and wellbeing, to prevent or delay crises. This includes promoting advanced care planning to enable appropriate responses should a crisis emerge.

These exciting roles demand excellent team working and communication skills and an innovative and committed approach to providing high quality, proactive patient care, within an integrated health and social service environment.

We have an extensive range of Advanced Practitioners, Clinical Specialists, Professional Leads and Non-Medical Prescribers across a wide range of specialisms that actively support clinical colleagues within our organisation. Our well established, structured Clinical Supervision and In Service Training programmes contribute to the learning culture which develops and challenges thinking and practice. As a Band 6 Physiotherapist you will hold a supervisory role for other members of the Physiotherapy team and would be expected to actively engage in this process.

You will need:

  • At least 3 years’ experience of working as part of an integrated multi-disciplinary team to manage complex cases

  • Excellent communication and IT skills.

  • Experience of working in people’s homes

  • Be highly motivated to provide evidence based practice.
