An exciting opportunity has arisen to be part of our Neighbourhood Team Service in South Worcestershire. You will be joining the evening team element of one of our dynamic and supportive Neighbourhood team to provide a 24 hour admission avoidance service to the population of South Worcestershire which encompasses both urban and rural areas.

 

The successful candidate will support nursing staff and deliver independently a high standard of nursing care for patients in their own homes. Ensuring that care provided maintains the individual’s privacy and dignity, values diversity and respects cultural and individual rights and choices.

Also as part of the role you will be supporting the Clinical triage of patients and supporting in an administration role, taking calls from patients and service users, and allocating the visits to team members. This will involve a 17:30 pm start at base. All other shifts will be from 18:30pm.

 

We are looking for candidates with good communication skills, a flexible approach and enthusiasm to work within the team. You should be a committed, highly-motivated and forward-thinking individual who is a good team player.

 

The successful candidate will also assist the team to develop and achieve team and organisational objectives. They will also participate in staff meetings and training & development activities as necessary to ensure their skills and knowledge are up to date.

 

Due to the nature of the post the applicant must be able to travel independently and have access to a suitable vehicle for business purposes. Whilst the base is identified, staff can be asked to work anywhere within the Business Unit or Service if necessary.

The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.
